Public Health Nursing 

Public Health Nursing is a specialty practice in the fields of both nursing and public health. Public health nursing is also often referred to as community health nursing. Our nurses wear a number of hats and serve many roles, but always with a focus on population-based prevention-focused services.

Please note that our clinics are nurse-run and we do not typically have a prescriber present. We do not offer physical exams or prescription services.

    • Provision of immunization records from Indiana's immunization registry
    • Facilitation for accessing your MyVax Indiana account to view immunization history
  • Reportable Communicable Diseases
    • Surveillance, monitoring and management of reportable conditions to prevent more people from being infected
    • Expert resource for providers seeking guidance on reportable conditions
  • School Liaison 
    • We offer public health nursing resources and support for school and childcare providers.
    • Facilitation of health education programming and resources.
    • Coordination for school-based services such as immunization clinics.
